BLESSED MARINA GLOBAL TRANSPORTATION - INDEPENDENT OPERATOR ICA

1. INDEPENDENT CONTRACTOR STATUS: The Operator is an independent contractor. There is no employer-employee relationship. 1099 tax reporting applies.

2. INSURANCE REQUIREMENTS: The Operator must maintain a minimum of $1.5M commercial auto liability coverage naming Blessed Marina, LLC as an additional insured.

3. VEHICLE STANDARDS: Vehicles must meet executive transportation requirements, be well-maintained, clean, and pass periodic inspections.

4. REGULATORY PERMITS: Operator is responsible for obtaining airport permits, decals, and operating licenses.

5. COMPENSATION STRUCTURE: Compensation is paid per completed trip in accordance with standard operator pricing logs.

6. TAX RESPONSIBILITIES: Operator is solely responsible for federal, state, and local self-employment taxes.

7. EXPENSES: Fuel, tolls, parking, cleaning, and maintenance costs are the sole responsibility of the Operator.

8. SCHEDULING: Chauffeur services are dispatched on a request basis. Operator has no mandatory schedules and is free to accept or decline rides.

9. PROFESSIONAL STANDARDS: Operator must maintain professional customer service standards and neat attire during client assignments.

10. BACKGROUND CHECKS: Ongoing background, criminal record checks, and MVR reviews apply.

11. INSPECTION: Operator agrees to make the vehicle available for safety inspections.

12. TERMINATION: This agreement may be terminated by either party at any time with written notice.

13. CONFIDENTIALITY: Operator agrees to protect client PII, itineraries, and Company proprietary business processes.

14. NON-EXCLUSIVITY: Operator retains the right to contract with other networks or booking agencies.

15. DISPUTE RESOLUTION: Governing law of the State of Georgia. Binding arbitration for unresolved commercial disputes.

16. INDEMNIFICATION: Operator agrees to indemnify and hold Blessed Marina, LLC harmless from liability or operator damages.

Consumer Reports Disclosure & Adverse Action Procedure (FCRA)

In accordance with the Fair Credit Reporting Act (FCRA), Blessed Marina Global Transportation discloses that consumer reports (including motor vehicle records, criminal background history, and safety profiles) will be requested during operator evaluation.

Adverse Action Procedure: If any information in the consumer report leads to application denial, we will provide you with a pre-adverse action disclosure, a copy of the report, and the Summary of Consumer Rights. You will have a reasonable period to dispute findings before final action is taken.

Written Summary of Chauffeur Applicant Rights

1. You must be told if information in your file has been used against you.

2. You have the right to know what is in your file from consumer reporting agencies.

3. You have the right to ask for a credit score.

4. You have the right to dispute incomplete or inaccurate information.

5. Consumer reporting agencies must correct or delete inaccurate, incomplete, or unverifiable information.
